Nothing on klmrecords.us is intended for protected health information. Do not submit patient names, dates of birth, medical details, or record requests through any form on this website. Information you give us. When you submit a form or contact us, we collect what you provide: your name, your office or company name, your email address, your phone number, and anything you write in the message field.
Information collected automatically. Like most websites, this site and its hosting provider record technical information when you visit, which may include your IP address, browser type and version, device and operating system, the pages you view, the time and date of your visit, and the site you came from. This happens through server logs and cookies.
We do not collect payment card information, financial account information, or government identification numbers through this website.
We do not sell personal information, and we do not share it with third parties for their own direct marketing purposes.
